HIV Testing in 2026: What a 4th Generation Test Adds
HIV testing has improved significantly over the past couple of decades, and the type of test matters — not just whether you get tested at all.
Older HIV tests looked only for antibodies, which the body doesn’t produce until some time after infection. A 4th generation test checks for antibodies and the p24 antigen, a piece of the virus itself that can appear in blood earlier than antibodies do — which means this type of test can detect an infection sooner than older methods.
Why the CDC recommends testing for everyone
The CDC recommends that every adult between 13 and 64 be tested for HIV at least once as a routine part of healthcare — not just people who consider themselves at higher risk. Many people living with HIV don’t know it, often for years.
A note on timing
Testing very soon after a possible exposure can still miss an infection that hasn’t yet reached detectable levels. If you’ve had a recent specific exposure, it’s worth asking a healthcare provider about the right timing to retest.
What to expect
No fasting needed. If your first result is reactive, the lab automatically runs a confirming test.
