This policy explains how SelectATest, a DBA of Jagers Enterprises, Inc., 5330 Bay Side Dr, Orlando, FL 32819. Email: labservices@selectatest.com (“SelectATest,” “we,” “us”) collects, uses and protects your information when you use selectatest.com.
Information we collect
- Order details: your name, date of birth, sex, email, mobile number and address, which the lab and ordering physician need to process your test.
- Tests and results: the tests you order and the results the laboratory returns.
- Payment: handled by our payment processor. We do not store your full card number.
- Clinic orders: if you order through a clinic, the clinic’s name and the details you choose to share with it.
- Website use: basic technical data such as your browser type and IP address, and cookies needed for your cart and sign-in.
How we use it
- To process your order, have it reviewed by an independent licensed physician, and send your requisition to the lab
- To deliver your results securely and let you know when they are ready
- To send sign-in codes and answer your questions
- To meet legal, tax and lab-reporting requirements
- To send offers by email, only if you opted in. You can unsubscribe at any time.
Who we share it with
We share only what is needed to provide the service: the independent ordering physician, the testing laboratory and collection site, our payment processor, and service providers that host our site and send our emails and texts. If you ordered through a clinic and chose to share results with it, we share them with that clinic. We may disclose information when required by law. We do not sell your personal information.
Text messages (SMS)
SelectATest sends text messages only about your account and orders: one-time sign-in codes, and a notice that your results are ready. A results text never says what was tested or what was found; it only tells you to sign in to your secure portal. We do not send marketing texts.
You receive texts only if you opt in, by checking the optional “Text me…” box at checkout, or by entering your mobile number to request a sign-in code. Message frequency varies (usually 1 to 3 messages per order). Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. Carriers are not liable for delayed or undelivered messages.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are not shared with any third parties, except the service providers that deliver our messages, solely to send them.
How we protect it
Sensitive order information is encrypted, results are available only after you sign in with a one-time code, and access is limited to people who need it to process your order.
How long we keep it
We keep order and result records as long as needed to provide the service and as required by law and laboratory record-keeping rules.
Your choices
You can ask to see, correct or delete your information by emailing us. You can unsubscribe from emails using the link in any email, and stop texts by replying STOP.
Children
Our service is for adults 18 and older. We do not knowingly collect information from children.
Changes
We may update this policy and will post the new version here with its date.
